We do not participate with Fidelis, Medicaid, CDPHP Essential Plan, Child Health Plus, CDPHP Medicaid, United Health Care Community Plan, United Health Care Medicare Solutions, Aetna Premier Network, MVP Medicaid, MVP Essential or Marketplace insurance plans.
If you have any questions about our participation with your insurance plan, please call our office or your insurance company before scheduling an appointment. If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.
